MEDICAID WAIVER PROGRAM |
|
For individuals receiving Medicaid benefits who are at risk of nursing home placement, the Home and Community Based Care (Medicaid Waiver) program is available. Clients enrolled in this program receive help from a case manager who works with the client to determine what help is needed to stay safely at home. The case manager will schedule services to meet client needs and will be there should any problems arise, service related or not. The case manager may schedule services such as homemaker services, personal care or respite care.
Homemaker services provide help with day-to-day household tasks such as cooking, cleaning, and food shopping. A homemaker can work in the home on average two or three times a week, but if conditions change, the case manager can adjust services accordingly.

Personal care services provide help with bathing, dressing, shaving and other special health needs. A personal care worker usually works with a client two or three times a week, but again the case manager can adjust this to meet individual needs.
Companion services are designed to provide support and supervision for clients. Companion workers may stay in the client’s home up to four hours a day and focus on non-medical care, safety and socialization.

Respite care provides someone to stay with clients while their regular caregiver is away from the home. The respite care worker may perform any of the tasks normally performed by the caregiver. Services can include either skilled or unskilled care. This is the only home and community based service with a limitation on the amount of service available.
Home Delivered Meals provide the client with at least one nutritious meal on a daily basis. Each meal provides one-third of the recommended daily nutritional values established by national standards. Clients may receive up to two meals per day.
